	We Know All About Forms

Our office will complete insurance claim forms for you – we know it can be a daunting task. Below is some general information.

PCP referral: Please determine if you need a referral from your primary care physician prior to your visit. Call before you arrive to confirm that all needed referrals have been received.

Insurance information: Please bring your insurance card. Also, let us know if your insurance information has changed and needs to be updated. You can call the office to confirm that AO is a provider under your coverage. If there is a co-pay for your visit, this should be paid at the time of your appointment.

New appointments: We ask that new patients arrive 15 minutes prior to their appointment.

Appointment cancellations: We request that appointments be cancelled at least 24 hours in advance.

Additional material: Please bring all pertinent X-rays, MRI's, CT Scan's, and EMG reports with you. Please have any relevant office notes and previous operative reports with pictures forwarded from your previous physician's office. Also please bring a list of your medications and the doses of each for your record.

Prescription Refill Policy: Routine medication refills will only be given during business hours, Monday through Friday, 8:30 a.m. to 5:00 p.m. Refills after hours and on weekends are done only for emergencies.
